Transcript Record Management not Recording All Info when Custom

Is there any way to get the description, student answer and correct answer to record in full when creating a "custom freeform question" like it does when using the default questions (i.e. multiple choice, true/false)? I am attaching a couple screenshots to explain what I mean.

The screen labeled "Description Using Template" shows the proper recording that we want in LMS, which happens when we use the default style questions. The screen labeled "Description (Custom)" shows what is happening when we use our custom design. We want to use our custom design that we've spent a lot of time creating and implementing, only to find that we aren't getting the right results. I am hoping there is a work around or just something simple that we are missing here.
