Video results in 404 not found during playback

I have a single-slide course where slide 1 holds a 10 minute video.
The video is inserted using INSERT>VIDEO>VIDEO FROM FILE
No error shown. Publish for HTML5 with modern player.
Doesn't play from our server. JavaScript console shows 404 not found for the mp4 file.
Search the published output for an mp4 and none are found. There is what appears to be a large video file but the filename ends with .mp4.qt which I understand is a QuickTime file format.
I'll file a case, but I hope someone here can point out if we've done anything wrong or if there is a workaround.
Last point... the mp4 file was generated by Storyline publishing a course into video format.

FYI - Articulate could not reproduce the problem although it failed every time on our Workstations.
The resolution was to create a new project and start over. Seems to publish as an mp4 now.
Buggy.
