What triggers 'Success' in LMS?

Hi, this is a continuation of a problem I had earlier this week.
I am testing a course in Scorm Cloud. The course is self paced, and we have a 'Course Complete' Trigger. In the Scorm cloud sandbox, this works - students are marked as 'Complete'.
There is however a 'Success' box, and for this, the student is marked as 'Failed'. Can anyone help me with where the LMS is pulling this information??
I do have a quiz in the course, but we don't have a 'pass' or 'fail' mark, it's just self check. I have marked the passing score as 0% because I thought maybe the LMS was pulling the Passing score from there, but no luck.

It is odd though that those settings are not working when a learner completes the course. With your permission, could share the .story file that is giving you some trouble? If you need to share it privately, please feel free to use this link here.
