This doesn't seem like a question about how well a learner has understood the subject knowledge, but rather how you want to track the learners. I'm not aware of any concrete rules for this. It may depend on how critical the training is and if stakeholders would like to see exactly where a learner is, or if they would like to analyze data to see what areas of a module learners slow down or speed up . Barring that, I personally like this:
Shorter courses -> report completion of course
Longer or more critical courses that require analysis -> Set Milestones
