evaluation questionnaires & printing certificates after exam completion

I want to insert an optional evaluation feedback questionnaire that the user should complete before getting his/her certificate. 

Am I correct in  guessing it should be built in a question bank type format and then drawn down ?

How would I feedback the completed questionnaire for evaluation ( am I feeding it back to the LMS )  ?

With regard to certificates, in the results slide, there is an option to print the result slide, how does it actually tell the printer ( I cant see anything obvious is in articulate ) ?

Also is there a way of utilising that results data in a bespoke certificate through articulate & printing off  ?

Or is the printing task all a function of the LMS and not articulate ?

Altan:

Question bank format - This should be fine. But a questionnaire is not really scored so you may want to consider using survey questions instead.

Feeding results back to the LMS - You can have any number of results slides but only one results slide can be sent back the LMS. This results slide can include any number of questions from anywhere in your course.

Printing - You can use this method to print the slide: Printing screen.

Bespoke certificate - You can customize a results slide however you want and print the screen or you can customize the report.html file: How to Create Certificates by Customizing the Report.html File or Example files: Generating a certificate from Storyline

Hi Steve

I think I'm better off, using an external evaluation questionnaire, such as survey monkey, providing the URL in a slide for users to click on, because as you say you can only feedback one set of results to the LMS.

I shall have a look at the printing and certificate creation links you provided and let you know how I get on.
