How to create a graded question?

Hi Team,
I want to create a question set which is kind of graded. Learners will be able to grade the question (A and B) from 0-5, 0 been the lowest and 5 been the highest (there are 15 questions in all). These questions will be divided based on few factors (for example: question 1, 2, and 3 will appear in one slide since they represent one factor. They will answer based on a set of guidelines.

At the end of the result slide it should show the option that has the highest number of response. There is no right or wrong answer. I have not yet tried the ‘Survey’ function in storyline.
