Hi! I LOVE this template that Nicole Legault created and THANK YOU so very much!! However, I need to add about 20 questions to this. I added just one to see how it would interact & to try to be proactive with any issues. For the life of me, I can't figure out what I'm doing wrong! I know it's probably something glaringly simple that I've overlooked, but if someone might be able to take a peek & help me out I'd be forever grateful!
