SCORM not reporting correct information to LMS

I have a Rise COurse exporting to SCORM 2004 4th Edition, tracking using quiz results. (*.zip file attached) I have used a divider in the course requiring the learner to complete all activities before proceeding.
I have a learner who has completed the course with 100% and passed the quiz with 100%, but the report to the LMS is that he has not completed some activities.
Using the LMS function I have masqueraded as the learner and completed all activities and it still reports incomplete.
Other learners are able to take and complete the course and are given credit in the LMS.

The fact that other learners are able to take and complete the course and are given credit in the LMS, except for this student, indicates that the problem is not with the Rise course.
What would cause Rise to report back to the LMS that there is no activity completion date? As I provided in my original message, I masqueraded as the learner and completed all activities, but still obtained the same result.
Hi Mark,
Rise may be sending the data to the LMS, but it's not being recorded by the LMS for some reason, e.g. a corrupt record.
As a LMS admin, I've seen this happen before.
