SCORM 2004 API Error when published to LMS

Hi,
I built out asurvey using Quizmaker 2013 with 6 simple questions. I published it to SucessFactors LMS using SCORM 2004 2nd edition.
When I review it on the LMS, the top SCORM navigation (Suspend, Continue, Exit) appear as well as the user interface navigation controls (PREVIOUS, NEXT and also FINISH) appear at the end.
When the user clicks FINISH on the UI, the module generates an API error trying to launch another session with the LMS.
What is the best way to handle this? Ideally, I would like to remove the SCORM 2004 navigation on the top frame and only keep the UI navigation.
I was told by our content integration service (iContent) that as long as I have the FINISH button on the UI, that it would continue to generate this error.
All adfvice would be greatly appreciated and I am attaching the quiz project file for your review.

I tested your content in SCORM Cloud and I am not seeing this result, which seems that this issue lies within your LMS.
You are welcome to test in SCORM Cloud for comparison if you'd wish, and here is my output.
Thanks Leslie for testing on the cloud. I got the same results as you did.
One last question, the content is recording completion of learning on launch. Is that a setting in Quizmaker or LMS?

Again, not seeing that result in SCORM Cloud, so sounds like something else you could address with your LMS
