All Answers Incorrect

I've created a short quiz and allowed users to review the quiz at the end. When the user reviews the quiz, all answers seem to be marked as incorrect in the bar at the bottom.

I have imported a couple of question slide plus the results slide into a new Storyline file, but still get the same results. This happens regardless of whether I'm running the file from my own hard drive or elsewhere.

To test, answer correctly "All of the above" to the first question, and choose the 2nd answer on the second question slide. Then review the quiz.
