Course with multiple scenes get marked complete without clicking Completed Button

Has anyone else had an issue where the completion flag is sent to the LMS (CSOD in my case) without the Complete trigger being activated in the SCORM 2004 course?
There are a couple of things going on here from what I can tell.
- The course tracking is set to only show the course as completed when using the "complete course" trigger (img_4).
- I have a course that unexpectedly goes from slide 1.1 to slide 6.1 (img_1) when clicking on the English radio button (img_2, slide 1.1).
- The slide containing the course completion trigger (img_5, slide 2.2) is bypassed.
- The course is marked as completed when the course is exited.
- When user relaunches course and clicks "No" to the bookmark "Would you like to resume where you left off?" (img_6), the course works as expected.
- No reports of other scenes having the same issue.

Since working with a Learning Management System can be very specific, I usually recommend testing the course in SCORM Cloud to rule out LMS issues.
However, I'm curious about your second item: "unexpectedly goes from slide 1.1 to slide 6.1 (img_1) when clicking on the English radio button (img_2, slide 1.1)."
If you're comfortable sharing your .story file either publicly here or privately through a case, we're happy to take a deeper look to see what's going on, and if that's the cause of the course completion.

You mentioned this is happening inconsistently. Do you mean that only a few users reported that the slide jumped from 1.1 to 6.1 when clicking the English radio button?
I noticed that the timeline for each certification slide lasts four minutes, and when it ends, it automatically jumps to the end slide without completing the course. Is it possible the user(s) stayed on slide 2.2 longer than 4 minutes, which automatically sent them to slide 6.1?
My test with SCORM Cloud did not reproduce this behavior when clicking through the course, as you can see below (I tested English, Portuguese and French):
If this is not a timeout issue, my recommendation is to reach out to your LMS to see if there's a setting on their end that could be causing this behavior.
