Creating a email in storyline

Hello,

I want the user to enter their name and any medical information required at the start of the course.  I want the user to be able to progress through the learning and once they have answered all questions correctly they then come to a slide which says "Thank you for completing the visitor induction" then they click a button.

I then want this button to email the entered text fields to a particular address saying "Mr J Smith" has completed his visitor induction.

And then in the body of the email it needs to stipulate what medical conditions (if any) he stipulated.  I work for a Mining Company and the LMS we use wont allow us to email administrators on different sites individually. 

Can anyone help with this or I am happy to provide more info.
