I'm using storyline 360 and for a quiz question I am asking the learner to drag icons into numbered squares, then click a custom SUBMIT button (that's the essence of it).
Every time, the answer is judged incorrect and the incorrect layer is displayed. I have tried the default submit button on the player - to no avail. I even though it could have been the template I had to use, it did not work when I created a simple drag and drop from scratch.
couple of things - 261MB for one slide is huge. I created a new project file and imported the slide in and it's now only 311KB.
The dotted line around your drop targets is the issue - this has been a bug since SL2. I made them a solid line and it works as expected. I've attached the file for you.

does the form view have the correct DnD matches set up?
can you share the .story file or even just the quiz slide. Use the grey 'add attachment' button at the bottom of the post to upload.

I am using icons and a text box (label) grouped together into a single object to drag.
I did try a similar question last night, an icon to drag into a shape (no fill, dotted line), using the default submit button - same result.

Yes, I did forget to remove the unused simulation recordings. Sorry about that.
The bug has been around since SL2? It never occurred to me.
