Draw from question bank - repetition?

I have a single question bank of 12 questions, with a request to draw 5, and a pass mark of 80%.

During initial testing, I thought I'd seen a repeated question (Q4 was also shown as Q5) but brushed it off as a glitch.

I've sent the course off to review and both reviewers have had the same.

There's a three year old thread where a bug was reported but no definitive outcome. Is there any known issue?

(My only other experiences of questions have either been the flat type in SL2, and an SL360 bank which drew five from five)

-The first thing I would recommend is to make sure you’re working on your local drive. Working on a network drive or a USB drive can cause erratic behavior. Please see the following article for more information and tips:

-If that is already the case, I would recommend applying the solutions in the article below to repair your Storyline app:
