Form to capture course issues

We are currently using Lectora and are considering a move to Storyline 2. I am trying to rebuild elements in our Lectora courses into Storyline 2, and one of the features I have not been able to figure out is how to create a form to capture any issues learners might find in a course (broken link, checkpoint error, image problem, typo/spelling).

In our Lectora courses, a learner presses a Submit Issue button, and a form appears that contains fields for name, email address, and comments as freeform fields. The form is pre-filled with the name of the course, and the page the learner was on when they clicked the Submit Issue button. When they click Submit Issue, the form is sent behind the scenes to an email address. Is it possible to recreate this in Storyline? I've attached an image of the current form.

Hi Michael,

The quickest solution would be to embed a form from a Google form as a web object.  Another option would be to include a short answer/essay question that the user could fill out and report that to an LMS or another tracking tool. Lastly what I see most users mention for a similar set up in the forums especially with emailing is to create the set up to run with Javascript, which is not something I'm equipped to support. 

Hopefully others in the community who have used that set up in Lectora will be able to chime in here as well.
