How to add multiple survey questions with Short Answers in one Survery Slide

You can use custom slide with any number of text entry fields.

Thanks to both for the quick response. Indeed it is needed to report the short answers to a Cornerstone LMS. I will try your suggestions.

Can you provided more details on how to do this exactly? I'm new at this.

Here are the steps that I think Dmitry took:

1. On a blank slide in Storyline, head over to the Insert tab
2. Select Text Box to add your question text and then hover the mouse over to Input
3. From there, select Text-entry field (under Data Entry), which will be where learners can add their answer
