Inconsistently giving a 0% score on successfully Completed tests

Our courses are set to Complete/Incomplete with a requirement of 80% to pass. I have one results page that scores a quiz and requires a score of 80% to pass, another results page for our evaluation questions that do have scorable questions with 0 points assigned, and a third results page that requires a combined score of 80% to pass. We are required to track and report all of our evaluation responses and our participants are required to answer all of the evaluation questions. We publish in SCORM 2004 in order to pull all of those results. Although most of the time the correct score and Completed status shows on the LMS transcript, other times a 0% and Completed status show. In the past, this has not been an issue, but it is something that we have noticed happening over the past few months. I have checked our course on the SCORM Cloud, and there were no issues. Our current LMS is Cornerstone. Has anyone else run into this issue?
