I am creating an exam and I need some assistance please. It is working exactly the way I want, except my incorrect feedback layer is not showing up. (See Question 1) I have it set up to allow 3 tries, but it will only show my Try Again layer. Can anyone take a look at my file and help me out with suggestions? I feel like I've tried everything! Thank you!
It's strange, if you change the number of attempts to 1 (to switch off the try again feedback) and then switch back to 3 attempts (to turn it back on) then everything works fine.
Could it have something to do with the feedback fields in the quiz form view being empty?

Storyline lets you decide how objects behave when learners revisit the same slide later. Choose from the following:
Resume saved state: Use this option if you want the slide to always remember its previous state. In other words, this option lets learners pick up where they left off when they leave the slide and come back to it later.
Reset to initial state: Use this option if you want the slide to always reset to its initial state when learners return to it. It'll restart from the beginning of its timeline, and interactive objects will return to their initial states.
Automatically decide: This is the default option. It means Storyline will decide whether to resume or reset the slide based on the objects it contains. Here's the logic: If the slide has only simple objects and/or audio but no interactivity, Storyline resets the slide to the beginning of its timeline. However, if the slide contains interactive elements, such as buttons or other objects that have a visited/selected state, Storyline resumes the slide where it left off previously.

Here is an explanation for the Revisit options in the slide properties.
I am having this same problem, and all the things I've tried (including the suggestions above) don't work for me. It used to work, but now it doesn't. Only the correct feedback shows up. When you answer the questions incorrectly, clicking the submit button for that slide doesn't do anything. I've tried changing the states of the slide. I've tried changing the number of attempts allowed (we want it to be unlimited, as it's technically not graded - however, we want to make sure that an incorrect answer does return the incorrect feedback with the correct answer). This is driving me crazy - a new employee just discovered this problem today and we cannot get it to work!
If you have unlimited attempts, they'll never see the "incorrect" feedback layer, only the try again layer until they answer correctly. I'm unclear on your statement about "make sure that an incorrect answer does return the incorrect feedback with the correct answer" ? Perhaps you could share a bit more information?
Sorry - I meant that in the text of the incorrect feedback, we tell them the correct answer. So should I change it to a single attempt, since I'm not doing "try again" feedback? I think we used to have "try again" and took it out - perhaps this is when everything went wonky!
Yes, you could change it to a single attempt and then they'd not have the "try again" layer and you will manually update the incorrect layer to include the feedback regarding what the correct answer is. But again with this set up they'll only be able to answer once.
This thread is a bit older and the suggestions above seemed to resolve it for the other users here, so if you're having difficulty still I think we'll need to take a look at your .story file. HELP! Here is a mini-version of one of our practices, with similar issues.
We have been using this type of coding for awhile now, but it is longer working as anticipated after we upload to Valamis (a new system for us.) Preview in the story or as a web page works exactly as anticipated.
The practice is set to 2 tries, with feedback given as needed. The text entry interactions are no longer getting the try again feedback when the answer is incorrect. A student enters whatever they want and they are moved to the next screen, as if their answer is correct. In theory, their incorrect text entry should provoke the feedback layer with the instruction to try again. If they get it correct, they are moved forward without feedback. The click-screen interactions are working as designed.
But, when they review the practice, they see their answers are incorrect and the feedback layer appears in the review as well as the reivew feedback layer.
I’m out iof my office ATM so can’t check the file but have you renamed the Correct/Inrrect slide layers at all? we did recently and exact same thing happened when published and viewed html5 output - skipped straight to next slide. We had to change wording back.
These attachments show what is happening in Valamis. Answers 1 and 2 are what I typed and got moved to the next screens, as if they were correct answers. Then the review clearly shows they were not correct answers.
Wendy, that is very interesting. Yes, we have renamed. I will take a look at that. Actually, I just checked this story. The feedback layers have not been renamed. They are: Review, Try Again and Incorrect. We are not using the Correct feedback on text entries, only on Click-screens and those are working. hmmmm. How in the world did you figure out that fix? Well, I am sorry to say that adding Correct feedback layers to the text entry questions did not fix the problem.
Just as a test Rory add back the Correct layer and see what happens. It was support that helped us figure it out - it had been working fine in SL2 and this was a file we upgraded to SL360 and she didn’t like our naming convention 😆

I had a similar problem and this helped fix it. I don't think it has anything to do with the fields being empty, just a bug in Storyline.
