Logo not displaying on menu in index_lms_html5.html

When I launch index_lms_html5.html, the logo on the top left of the menu is broken. This isn't a problem with the other launch pages.
Also, the bullets in the resources section are cut off in this launch page, but look fine on other launch pages.

You need to delete the logo , reinsert and publish again.

Any idea why the bullets are cut off in the Resources panel?
Sorry no idea, I have never seen the bullets before so shows how observant I am.
