Not being offered all the questions in a quiz

It seems that at least one of our modules, created in Storyline 360, is getting most of the way through the quiz, 26 questions from a bank of around 50, and jumping straight to the results screen. This looks like it may be a random number. I’ve managed to go all the way through with no issues, and then, when retaking it, I was dumped out at 22 questions. a colleague has had a couple of shots, where she was asked 24 or 25 questions, but never the 26. We also have a track of the question number that displays to the user, and when retaking the quiz, after failing, that will remain at the last question number give, so ’22 of 26’.
We're just moving to a new LMS, and are just testing these files out prior to our April go live, and wondered if anyone here has had similar experiences, and if there are any suggestions? The file works fine in our old LMS.

Are you able to replicate the behavior in SCORM Cloud? If so, then with your permission, I'd like to take a look at your project file to investigate what's happening. You can share it publicly here, or send it to me privately by uploading it here. I'll delete it when I'm done troubleshooting.
Hi Leslie, sorry I didn't get back to you in this, it's been stupidly busy here. We managed to resolve the issue of the banks sending you out too soon, but we still have a question regarding the numbering of questions. When you take a quiz, but fail, and are asked to retake the only the questions you get wrong, the count does not show the original question numbers, rather it just shows the last number on all slides being retaken, (20 out of 20 for example). we've got a file uploaded to Articulate Review, but I'll send you the source file privately if that's OK and you are still able to take a look?
