Quiz Scoring errors

Hello,
I need some assistance on identifying an error with a SCORM. We have run into a problem and we cannot exactly pinpoint where the error is occurring. It is specific to the quiz that was created in the SCORM. We have found that when a user selects the Retry Quiz option, the scoring becomes problematic. It is not always sending interaction data back to the LMS and when it does, the questions are appearing out of order when we review our reports. However, sometimes it is intermittently capturing the score. We have been urging users to begin a new attempt of the SCORM rather than selecting retry quiz, but we would still like to know what could be happening with the SCORM to cause this problem.
I've attached the manifest file, but I'm not sure if this is where the error is. Please advise.

Hello Michelle!
Have you tried testing this on the SCORM Cloud to see if you get the same errors? Thi sis the industry standard for testing and can let you know if it is on the LMS end or not.
