Reporting incorrect scores in LMS

Have you tried uploading to Scormcloud (industry standard LMS) and see if it works correctly there. If not, you might like to give more details about the LMS you are using so that people can offer some assistance.

I have tested the course in SCORM cloud (SCORM 1.2), it works fine there. 

I will go through  the blog and get back in case the issue is not resolved.
