We have this handy article that you can check out which explains how you can setup your quiz so that only incorrect questions are taken on a retry attempt:
You'll want to make sure that your question slides all have submit interaction triggers, and that you have a Results Slide that also has a "Submit Results" trigger. A safe location to place the "Submit Results" trigger is in the Results Slide itself.
If you have all of this setup, and are still encountering issues with scoring, try testing your course in SCORM Cloud to see if the behavior is specific to Moodle. If you are able to observe the behavior in SCORM Cloud, open a case with our support team here so we can take a closer look at your file.
