Review quiz - feedback incorrect

I have a course that has been in our LMS for almost 3 years- developed in SL2. The only changes we've made to any of the questions in the final exam is adding to the "correct responses" list for fill-in the blank questions. If the learner puts a correct response, the question is marked correct (and reports as complete to the LMS), and the feedback slide shows correct.  Problem occurs at completion of quiz if the learner goes back to use the "review the quiz" feature. The feedback shows incorrect- see screenshot. Any ideas as to what is making the feedback show incorrectly via "review quiz"?  The scores are being reported correctly. The fill in the blank questions are not marked "case sensitive".  Here are a few screenshots-

Sorry that you've run into an issue reviewing your course, but thanks for such a thorough explanation and screenshots.

I was able to recreate the issue you are experiencing and it comes down to the Slide Properties on your question slides:

Be sure that you have the When revisiting property set to either Automatically decide or Resume Saved State.
