Sending Quiz Score to SF Learning (Plateau)

Is there a secret combination of settings that I need to follow to get quiz scores returned to SF Learning?

I have a separate course and quiz that I'm migrating from SumTotal to SF Learning.   In SumTotal everythings works fine.   In SF Learning, the course works fine, it's marked complete when the learner views all pages, but the quiz never sends the score or completion status back the LMS.

I'm publishing the two content objects as SCORM 2004 Rev 3,

The course marks complete using passed/failed when the learner views all pages

The quiz is supposed to mark complete also using passed/failed when the learner gets a score of 80%.  I'm submitting quiz results as a trigger when the results page is loaded.   I also have it set in the tracking tab as the student is marked complete based on the same results page.

Any suggestions or steps that other people follow to get quiz scores back to SF Learning?
