I have a Retry Quiz button on the Results slide if the learner doesn't pass the assessment. This takes them to the slide before the question bank. Retaking the quiz, I am presented with only four of the seven questions in the bank. Allow user to retry quiz is selected. In Draw Questions from Bank, All questions is selected. Draw Questions randomly is selected. Should it be? For Include in Shuffle, I have tried Randomly and Always. What am I missing?
Well....it's not working, but giving conflicting results. It works for me in my LMS (Totara), but not for another tester. The Web output running in the browser also only presents four of the seven questions.
Thanks for including your file in this thread. I published it for Web here and for LMS here, and each time I did a quiz retry, I saw all 7 questions. This can be a tough one to pinpoint since you're not able to reproduce the problem, and I'm also not seeing it on my end. Do you know if the other tester was testing the Web output locally or on a web server? Viewing a course locally may cause you to encounter security restrictions from the computer, web browser, Flash Player, and network that'll cause various features of your content to fail.

In Form View, the Attempts field shows 1 for each question and is grayed out (not selectable).
This project started working as designed. I can't explain why.

Maybe I should delete the question bank and re-create it.
One more piece of info. I inherited this project and don't know its history. It may have been a Storyline 1 project originally.

Thanks Alyssa. I opened a ticket...although it's now working correctly in two LMS's. There may be something I missed.
