Hoping someone can assist. SCORM results for most learners are being recorded correctly within Moodle, however, there are a handful of learners coming through where the result is being recorded incorrectly, for example one user received 91.66%. When I look at his SCORM attempt results he has correctly answered all the questions. Grade to pass has been set in both Storyline and in the Gradebook. I cant work out what's happening..

Have you tried another LMS like SCORM Cloud – an industry-standard tool for testing SCORM output – to see if you experience the same behavior there? If the grades match when testing there, it may be a good idea to reach out to Moodle directly for additional assistance.
Do you notice a trend among the learners with the conflicting grades – are they using the same browser?
