Track via LMS how learners answer a question

I am trying to track responses for a question slide. I want to be able to track learners who select No. The way I've designed the lesson, learners can try again if they answer No the first time through. However, when I added a Results slide, the question slide does not reset and allow the learner to actually try again. Do you have any recommendations on how to set this lesson up so I can track learners who select No the first time through?

I've attached two versions of the lesson. The "Menu" lesson shows how I want learners to be able to complete the lesson. The "TestQ-ResultsSlide" was my attempt to get the lesson to score on the LMS - but it's not working the way I hoped it would.

Thanks for sharing a copy of your project files!

Since your main goal is to allow your learners to retake Slide 1.16 if they initially get it wrong, you'll want to add a trigger that is responsible for resetting the 'Results Slide' during a retry. 

It's important to place the 'Reset results' trigger before the 'Jump to slide' trigger in the slide order to allow the slide to reset the results before navigating away from the slide.

I wanted to capture any No responses to the "Day Two - Is provider available?" slide, so I created a duplicate slide and used it for navigation only.
