Sounds like you are using Storyline 2. I'm not aware of an issue based on that description, but that certainly is not the experience we want you to have.
This is happening once the course is published?
Do you have a .story file that I could take a look at?
It looks like your email signature came through when you replied via email. You can remove that if needed and here’s a quick Peek video if you need help.

It appears that this occurs if you have a trigger in your second slide that will go back to slide 1.1 when the user clicks the "Prev" button. We've verified that this issue was introduced after we release the latest update for Storyline 360. I’ve logged it as a possible software bug.
Quick update - we just pushed out a hot fix for this issue where a video on the first slide of a course wouldn't always play in HTML5 output. You'll see the release notes here.

We are having the same issue.
Update 2 now breaks our mp4 videos whereas SL3 Update 1 was perfectly fine.

Based on Andrea's mention of the latest update being on 5/10, I assume she's using Storyline 2.
If you are seeing a similar issue with Storyline 3, we would want to take a look at that as well. Could you share your .story file with our team here?
I use storyline 360.
