Why is my course showing as failed?

Hoping someone out here can assist.  My company uses oracle people soft as our LMS vendor.  I have published a course with a minimum slide view for completion and to record either incomplete or complete.

I have tested the course successfully and the status shows complete in my learning history, however upon the re-launch screen it says failed.  This does not change my completion status, but is preventing me from rolling out the course as our IT department says there is something wrong.

I've developed courses previously without this issue, am I doing something wrong in the settings that could cause this to occur?

Any insight would be appreciated.

Hi S ID,

It just within the actual course itself that states it was failed, and not in the LMS reporting, correct? Or is it changing the status to "failed" when viewing the course details in the LMS?

Have you tested your course in SCORM Cloud?
