Courses Failing, but not Passing?

Hi Heroes!
Has anyone ever encountered instances where their courses will track a 'Fail' % AND status, but not a 'Pass' % OR status?
I'm using the Pass/Fail option for reporting, and publishing to SCORM2004 for our LMS (Halogen). There have been at least one instance where the individual should have passed the course, but their status and results were never updated within the LMS. However, we have had multiple individuals fail the course, and have their status and results updated properly.
Has anyone ever heard of this happening? Are there known solutions? Thank you as always for sharing your expertise and experience!

We have been struggling with this problem for about 3 months now.
We have combed through all our logs, republished, republished, etc., etc.
And no resolve.

Hi Donna,
Thanks for letting me know I'm not alone in this problem. I should have mentioned that this is only one of multiple courses programmed in this fashion, and it is the ONLY one we are having troubles with. Are experiencing a similar one-off issue, or are all your courses behaving this way?
