We use Embed.ly to embed rich media in Rise courses. When I dropped your form link into the Embed.ly Embed Code Generator, it appears that Embed.ly is removing the pre-filled text.
If the pre-filled text is something you'd really like to keep, could you create a hyperlink or a button to open the form in a new window?
Could you please let me know how the Embed.ly code is inserted into a Rise block. I take it this is inserted into an Embed block, however the Embed.ly code is not in an iframe.

Sadly, this is still affecting embed links. I'm trying to embed unified course survey into several courses and pre-fill course name. However, when the form is embedded into the Rise course the pre-filled field is empty.
Did you escalate this problem to Embed.ly? What's their reason for removing parameters from the URL?
I have this example I created with Google Forms, though it doesn't have prefilled fields. We're happy to sign an NDA in a private case if it helps. Otherwise, I recommend removing parameters to test if any of them are causing an issue.

This is the workaround I used. However, this means the form is launched in a new tab. This means extra instruction / warning before and after.
I would rather have the user stay on the same page!

Trying to paste <iframe> produces an error (attached).
As for survey URL - I'm afraid I cannot send it over, but this problem can be reproduced on any Google Forms survey.
Got it - I wonder if there's an issue with the parameters in that embed code. We use this general guidance for embeds in Rise 360.
