Developmental Network Mapping Exercise Help

I am trying to create a networking exercise for a program I am involved in. The goal would be to have the subjects fill out the necessary fields in an AS2 program, and when complete have the data either populate a flow chart or simply compile the info into one place when complete. It seems to me that all the options for quizzing or surveys ends up being either a "correct" or "incorrect" and we are not trying to quiz our employees, as there are no right or wrong answers.
For example, I am trying to start with a slide that asks a question such as "Who are 5 important connections you have and what are their roles in your professional life?" After answering that question, they will be asked a sequence of questions regarding their answers. Is this possible?

Hey Kiki. The simple answer is yes you can do this. I looked for an article that covered the two things you'll need to complete this and this one covers both (using text entry fields to collect data, and using variables to output that data).
