need help- reporting to LMS when having multiple questions in 1 slide

Hello everyone.

Can someone please tell me how can I report the answers the user submit- to LMS when I have multiple questions on each slide ? (6 in the pic above for example).

I have tried using a quiz , but it is limited to 1 question per slide.

Is there another way ?

i have attached storyline slide with a single question quiz so that u would be able to change to mulitple questions quiz if its possible.

This is not a built-in feature, but something that would need to be created.
