I have built summative assessments using a mixture of graded, survey and freeform assessments. These are scored on my results slide with standard storyline variables and triggers.
My course is published in SCORM 1.2 and as Passed/Failed. Both Cornerstone and LSO are not able to score and report on my results.
What might cause this problem? I have checked and re-checked all my publishing settings and am positive they are correct?

Actually now that I reread your post - you must publish in SCORM 2004 to be able to report on SCORM-based tests in Cornerstone. SCORM 1.2 courses are not included in the Standard Reports around SCORM. Also, another thing to be aware of is that Cornerstone cannot report on a single user's test score for a SCORM course unless you constrain the report criteria to a single user.

Not sure if it would make a difference, but Cornerstone "prefers" SCORM 2004 3rd edition over SCORM 1.2. Might be worth testing using SCORM 2004.
